Photography, Video, and CCTV Release
Participants in any activities at NW Juggernaut MMA LLC may be photographed or recorded for promotional and advertising purposes, including social media and other platforms. By signing below, the participant consents to the use of such media without compensation by NW Juggernaut MMA LLC in editorial, promotional, and advertising materials.
Additionally, NW Juggernaut MMA LLC utilizes CCTV security cameras for the safety and integrity of members and staff. Footage is recorded and stored for security purposes and may be reviewed by management in the event of an incident such as theft or property damage.
WAIVER AND RELEASE OF LIABILITY
Assumption of Risk
I, the undersigned, understand that participation in physical training involves inherent risks, including but not limited to: falls, equipment failure, and injuries such as strains, sprains, fractures, or even death. I acknowledge that these risks may arise from my own actions, the actions of others, or equipment use. I voluntarily assume full responsibility for any injury, harm, or death that may occur while participating in activities at or under the direction of NW Juggernaut MMA LLC. I confirm that I have no physical impairments, injuries, or illnesses that would endanger myself or others.
INDEMNIFICATION
I acknowledge the risks involved in activities offered by NW Juggernaut MMA LLC and accept financial responsibility for any injury I may cause to myself or others due to negligence. Should NW Juggernaut MMA LLC or its representatives incur attorney fees or costs to enforce this agreement, I agree to reimburse them for such expenses. Furthermore, I agree to indemnify and hold harmless NW Juggernaut MMA LLC, its owners, agents, employees, and volunteers from any liability arising from injury, death, or property damage caused by my actions while participating in activities at the gym or external training locations such as parks, playgrounds, or other facilities.
PARENTAL INDEMNIFICATION (For Minors)
If signing on behalf of a minor, I agree not to file any claim against NW Juggernaut MMA LLC, its instructors, staff, students, or affiliates for injuries sustained by my child, except in cases of criminal acts (for which only the perpetrator shall be liable). I also agree not to encourage or support such claims. Additionally, I will indemnify the gym and its affiliates for any legal fees, judgments, or costs resulting from a breach of this agreement.
NOTICE AND CONSENT TO INSTRUCTORS
NW Juggernaut MMA LLC employs experienced and highly trained instructors to lead classes. Instruction may be provided by the head instructor, assistant instructors, senior students, or guest instructors at the gym’s discretion. I acknowledge that I may not always train under my preferred instructor but will respect and learn from any assigned teacher. I understand that my safety is my personal responsibility, regardless of the instructor assigned to a class.
GENERAL RELEASE OF LIABILITY
In consideration of the risks involved and my voluntary participation, I release NW Juggernaut MMA LLC, its owners, agents, employees, and volunteers from any and all liability, claims, demands, or actions arising out of my participation, including those due to alleged negligence. This release is binding upon me, my heirs, representatives, and assigns. If any portion of this agreement is found to be invalid, the remaining provisions shall remain in full legal force and effect.
If signing on behalf of a minor, I grant permission for NW Juggernaut MMA LLC personnel to administer necessary first aid and, if required, seek medical care or transport the child to a medical facility.
DURATION OF AGREEMENT
This document is effective from the date signed and remains in effect indefinitely. The terms apply retroactively to any training or visits to NW Juggernaut MMA LLC prior to the date of signing.
SIGNATURE
I have read and understand this waiver, release of liability, and indemnification agreement. By signing, I acknowledge that I am waiving significant legal rights.
